Before your MyTutor lessons can get started, we’ll need you to complete our Pupil Details Form. This is the document we’ll use to collect all of the information we need about pupils so that we can book in their lessons and match them with tutors.
To download your copy of the form, head to your New 1-1 or 3-1 Programme Page on your account and click the button which says ‘Download blank template’
The Pupil Details Form has four sections which we’ll need you to complete.
The first section is where you’ll fill in pupils’ names, their year group, an email address and gender. In cells which say ‘Please select’, choose an option from the drop-down menu. This helps us to efficiently retrieve data from your form in the right format.
The second section only needs to be filled in for pupils whose programmes are being supported by the National Tuition Programme. For their evaluation, the NTP will need Unique Pupil Numbers (read about this here - https://www.gov.uk/government/publications/unique-pupil-numbers), dates of birth, and the Pupil Premium and SEND status of pupils.
In the third section you can tell us which subject the pupil would like for their MyTutor sessions and the day and time these should be scheduled.
The final section asks you to include some information about the level pupils are working with. We ask for current grades, target grades and exam boards so that we can match pupils with suitable tutors. Any information which you include in the final two columns will be passed on to tutors to help them to plan their lessons.
If your pupils are having 3:1 lessons, the form is exactly the same, except it’s split up into groups. Pop each set of three pupils you want to work together into each group.
